Apicoectomy Surgery in Jersey City, NJ

An apicoectomy is a minor surgical procedure that removes the tip of the tooth root, or apex, along with any infected or inflamed tissue, and seals the root end to prevent reinfection. At Dental Associates of Jersey City, our team performs apicoectomy procedures using operating microscopes and CBCT cone beam imaging to ensure precision while preserving as much healthy tooth structure as possible.

This procedure is typically recommended when a root canal treatment has not fully resolved an infection and retreatment is not possible or has not been successful. Our practice, led by Dr. Andriy Yuzvyak, brings a patient-centered approach to endodontic surgery for patients across Jersey City and the surrounding communities.

What Is an Apicoectomy?

An apicoectomy, also known as root-end surgery or periapical surgery, is the most common form of endodontic surgery. It is recommended when inflammation or infection persists in the area around the root tip after a root canal procedure, and when the problem cannot be resolved through nonsurgical treatment.

Unlike a root resection, which involves removing an entire tooth root, an apicoectomy removes only the very tip of the root — making it the more conservative surgical option when the problem is confined to the root apex. According to the American Association of Endodontists, an apicoectomy may be needed when inflammation or infection persists in the bony area around the end of a tooth after a root canal procedure.

When Is an Apicoectomy Needed?

Our team may recommend an apicoectomy when one or more of the following apply:

Persistent infectionPain, swelling, or infection that continues after root canal treatment
Retreatment not viableWhen the tooth has a large post, is part of a dental bridge, or retreatment has already been attempted
Complex root anatomyRoot structure that prevents thorough cleaning through nonsurgical approaches
Dental traumaCracked or fractured roots that cannot be stabilized through other treatment

How the Apicoectomy Procedure Works

The procedure is performed under local anesthesia and typically takes under 90 minutes. Here is what you can expect at each stage:

1AnesthesiaLocal anesthesia is administered to numb the area and ensure your comfort.
2IncisionA small incision is made in the gum tissue to expose the underlying bone and root tip.
3Tissue removalInfected or inflamed tissue is removed from around the root tip using precision microsurgical instruments.
4Root-end sealA few millimeters of the root tip are removed, and the canal is sealed with a biocompatible filling material to prevent reinfection.
5SuturesThe gum tissue is sutured closed. Depending on the suture type, they may dissolve on their own or be removed at a follow-up appointment.

All procedures are performed under high magnification to ensure precision throughout the process. Over the following months, the bone around the treated root end heals naturally, restoring the integrity of the jaw and the function of the tooth.

Comfort and Sedation Options

Our team prioritizes your comfort throughout the procedure. In addition to local anesthesia, sedation options are available for patients who experience dental anxiety. Our team will review your situation and discuss the most appropriate approach before your appointment.

What to Expect During Recovery

Most patients return to normal daily activities within one to two days after an apicoectomy. Some swelling and discomfort around the treatment area is normal and typically resolves within 48 to 72 hours.

First 24 hoursApply cold compresses in 15-minute intervals to manage swelling.
DietAvoid hard or crunchy foods for several days following the procedure.
Oral hygieneBrush gently near the surgical site during the first few days of recovery.
Suture removalReturn approximately one week after the procedure for suture removal and a healing check.
Pain managementOver-the-counter or prescribed analgesics as directed. Contact our office if pain is severe or worsening.
