What Is Gum Disease and How Does It Start?

 

Gum disease, clinically known as periodontal disease, begins with bacteria. The mouth constantly produces plaque, a sticky bacterial film that coats the teeth. When plaque is not consistently removed through brushing and flossing, it hardens into tartar, a substance that cannot be removed at home and requires professional cleaning to address. As tartar accumulates along and beneath the gumline, it triggers an immune response that manifests as inflammation, the hallmark of early gum disease.

According to the CDC, periodontal disease is a chronic inflammatory condition that can progress from mild gum irritation to serious destruction of the bone and tissue supporting your teeth. What makes this condition particularly tricky is that it rarely causes significant pain in its early stages, meaning many people are unaware they have it until the disease has already advanced.

Gingivitus

Stage One: Gingivitis

Gingivitis is the earliest, and only fully reversible, stage of gum disease. At this point, bacteria have irritated the gum tissue, causing redness, puffiness, and bleeding during brushing or flossing. Critically, no bone loss has occurred yet, which is what makes this stage treatable without permanent consequences.

Common signs of gingivitis include:

  • Bleeding gums: noticeable during brushing, flossing, or even eating firm foods
  • Redness or swelling: gums that look darker pink or appear puffy along the gumline
  • Tenderness: gums that feel sensitive or sore to the touch
  • Persistent bad breath: caused by the bacteria accumulating around the gumline

With consistent professional cleanings and improved at-home hygiene, gingivitis can be fully reversed before it causes lasting damage. This window of opportunity is precisely why routine dental visits are so valuable.

Stage Two: Mild to Moderate Periodontitis

When gingivitis is left untreated, it transitions into periodontitis, a condition that involves bone loss and cannot be reversed. In the mild stage, bacteria have migrated below the gumline and begun affecting the bone that anchors your teeth. The gums start to pull away from the teeth, forming pockets that trap additional bacteria and become increasingly difficult to clean at home.

Moderate Periodontitis

As the disease progresses to moderate periodontitis, the bacterial infection erodes the connective tissue, ligaments, and bone more aggressively. Patients often notice persistent bad breath, visible gum recession, and sometimes pus around the gumline. This is the stage where non-surgical gum treatment becomes an important intervention. Scaling and root planing, sometimes called a “deep cleaning,” is one of the most effective approaches at this stage, allowing us to remove tartar from below the gumline and smooth the root surfaces to discourage bacterial reattachment.

Stage Three: Advanced Periodontitis

Advanced periodontitis represents the most serious progression of the disease. At this stage, significant bone loss has occurred, often causing teeth to become loose, shift out of alignment, or require extraction. Patients may experience pain while chewing, changes in bite, and visible changes in how their smile looks as teeth move or gaps form.

Treatment at this level typically involves more intensive periodontal therapies and, in some cases, surgical intervention to restore gum and bone structure. Scaling and root planing remains part of the therapeutic approach, though it is often combined with additional procedures depending on the depth of the pockets and the extent of tissue loss. Left untreated, advanced periodontitis is the leading cause of tooth loss in adults.
